New continuation form for imprest checking accounts

Cash Management has posted a new form from Bank of America for the signature cards for imprest checking accounts. The new form, 306.1.3f – (Bank) Deposit Account Documentation Card Continuation, is a continuation of form 306.1.2f – (Bank) Deposit Account Documentation Card, and is to be used if more than two departmental signatures are needed 306.1.2f.

Please begin using this new form for this purpose instead of sending multiple copies of 306.1.2f to Cash Management. Please email cash_management@unc.edu if you have any questions.
