Please be aware that the deadline for submitting Fall 2012 Graduate Payroll Deduction Forms to the Cashier’s Office is 4:30 p.m. on Friday, Sept. 7, 2012. The original, signed form must be received in the Cashier’s Office. Faxed or emailed forms will not be accepted. Authorized deductions will be taken out of the monthly paychecks issued at the end of September, October and November.
**Also note, if a student is receiving a non-service award or other financial aid, the student no longer qualifies for payroll deductions. The fees will automatically be deducted from the non-service award or financial aid.**
The Graduate Student Payroll Deduction Authorization Form is the first form listed under the “Departmental Forms” section of forms page on the Cashier’s Office web site.
