Rosa Nolen, FRS Coordinator June 10, 2008 Finance has released a new version of the “Consolidated FRS Single User Online Access
Request” form used to request online access to financial systems. The new version of
this form also has a new name. The “Consolidated Financial Systems Access Request”
form is in PDF format and can be completed online.
The revised form is available on the Finance web site under the “FORMS” tab Finance -
Finance Forms - Most Used Forms. You will still need to print the completed form,
obtain the appropriate signatures, and send to the FRS Coordinator for processing.
Please use the new form for all new online access requests and changes. If you have
saved a copy of the old form please delete it and use the Finance web site to access
the current version. We anticipate future changes to the form, so ensure you have
the most current version by always accessing the form via the Finance web site.
